Forms necessary for youth group participation are:

1. General Permission Form 

2. Participant Rules Agreement

3. Participant Photo Release Form

Once all forms are completed and signed, you can hand them in at a meeting, at Taylor's office, through the Parish Office, or by scanning them into an e-mail.
